REQUIRED DOCUMENTS BY THIS CONSULATE GENERAL
01. When a Sri Lankan citizen in the UAE is deceased, the Consulate General and/or NOKs(legal relatives) should be informed in the first instance.
02. This can be done by any person who knows the information about the death. Hence, if the Consulate General is informed regarding the death, the letter/s issued by the Court, the Police, the health authorities of the UAE should be submitted. Along with such documents, please also make sure to produce the documents to verify the Identity of the deceased such as Passport, Emirates Identity card or Sri Lankan National Identity card.
03. The Human Remains could either be repatriated to Sri Lanka or buried in UAE. If the Human Remains are to be buried or cremated in UAE, approval should be granted by the relevant UAE authorities. Whatsoever the above procedures are followed, the consent of the NOKs(legal relatives) should be given through a formal affidavit.
04. If the declarant of the affidavit resides in Sri Lanka, the affidavit should be signed at the Ministry of Foreign Affairs, Colombo, or Divisional Secretariat relevant to his/her resident address. If she/he is a resident in the UAE, such affidavit could be signed at the Consulate General office(upon presenting formal evidence to prove relativity to the deceased)
05. If the death occurred due to a suicide, an accident or a homicide, the evidence must be obtained based on the documents issued by the entities such as Court, Police or the health authorities. Such documents may subject to vary depending on individual case.
06. The employer is obliged to repatriate or bury the HR of an employee who worked under a valid employment agreement at the time of the death.
07. If the employer entrusts the proceedings related to the death to a specialized company, it should be informed formally to the Consulate General (Through a letter addressed to the Consul General by the company where the deceased worked at the time of his demise in letter head format)
08. It must be strictly ensured that no burial or cremation should be done in the UAE without obtaining No Objection Certificate from the Consulate General office.
09. In order to carry out the burial or cremation in UAE or to repatriate the Human Remains to Sri Lanka the following documents should be produced to the Consulate General office for obtaining the No Objection Certificate.
- a. Death certificate issued by the health authorities of the emirate concerned with a copy of English translation. (Specimen 01)
- b. Death notification (issued by the hospital/Specimen 02)
- c. The letter “To whom it may concern” issued by the Ministry of Health & Prevention of the emirates concerned. (Specimen 03)
- d. Medical certificate of the death (issued by hospital./Specimen 04)
Please be noted that the cause of death must be indicated at least in one of above a,b,c or d)
- e. Visa cancellation papers (Visa cancelation should be done by UAE immigration/Specimen 05)
- f. Original Passport of the deceased (should be canceled by the Consulate General of Sri Lanka)
- g. The documents mentioned in Para 05 above.(Specimen 06)
- h. If the deceased is a company worker his/her Labour Identity card should be cancelled.
- i. Clearance letter from the UAE employer of the deceased, stating the details related to his salary dues, Gratuity and Compensation etc.
- j. The details of the Cargo service and Airline.
- k. Any other document/s if necessary
