CHANGE THE NAME IN THE PASSPORT AFTER MARRIAGE

This service facilitates the endorsement of a name change in a Sri Lankan passport following marriage. Upon submission of the required documents and approval from the Controller General of Immigration & Emigration in Sri Lanka, the alteration will be made as a handwritten endorsement on the Alteration and Observation page of the passport, bearing official logos and seals.
- Duly filled Application [Download Application Form “E”]
- Written request from the applicant requesting the name change
- Current Passports of both Husband and Wife (originals)
- Copies of the Passports of both parties
- Marriage Certificate with a copy – Original document in Sinhala or Tamil is required. Translations or photocopies will not be accepted.
- Consent Letter from the Husband
- Birth Certificate of the applicant – Original document in Sinhala or Tamil is required. Translations or photocopies will not be accepted
Note:

The name change will be endorsed only upon approval from the Controller General of Immigration & Emigration in Colombo, subject to the submission of all relevant documents.

The alteration will appear as a handwritten entry on the Alteration and Observation page of the passport, accompanied by official stamps and logos.

Husband’s written consent is mandatory if the wife wishes to adopt his surname in the passport.
